SUMMIT TRIATHLON TEAM CHAMPIONSHIP SERIES (LEG 2) - Online Registration

Registering for a Tandem or Relay entry? This a team entry which is either a two-man(Tandem) or three-man(Relay) team, both in one category. Each member of the team must register individually but with consistent "Team name" and "Category" with other members.

Fund transfer via Union Bank online banking is also accepted.

STEP 2: Send transaction identification number immediately either via text, +63.933.925.6343 or email to info@sugbutriathlon.com for verification.

Where do I find the transaction identification number?
- Deposit slip: It is prefixed with "UB" followed by the numbers.
- Fund Transfer: It can be found in the Cyber receipt.

Please take note that step 2 will finalize your registration. No transaction ID, no registration. It may take one to two days before your name will reflect in the Participant List.

WAIVER / RELEASE FORM

In consideration of my entry, I hereby certify that I am a participant of this event with the following conditions:

1. I'm physically fit to join the event, has sufficiently trained for the same, and is not suffering from any illness or disease that will cause injury or death by participating for the same;

2. Has not falsified any document nor furnished the Summit Natural Drinking Water and SugbuTriathlon with any false information which may be material to the Organizers and Co-Sponsors in determining his/her capacity as a participant to engage in the activities of the Event and;

3. Grants the Organizers/Sponsors full permission to use his/her photographs and/or motion pictures, recordings or other records of the Event featuring the undersigned for any legitimate purpose, including commercial advertising.

4. I represent and warrant Summit Natural Drinking Water and SugbuTriathlon that I have full legal authority to complete this event registration on behalf of myself and/or any party I am registering for.

Should the undersigned be made aware of any such circumstance relating the foregoing from time of execution hereof, the undersigned undertakes to immediately inform the Organizers/Co-Sponsors of this matter and furnish the Organizers/Co-Sponsors with all the information the latter may require.

In view of the foregoing, the undersigned, for himself/herself, and for his/her heirs, agents, executors, and administrators, hereby waives and releases any and all causes of action in law or equity, rights, and claims for damages that he/she may have against the Organizers/Sponsors, their representatives and successors, and holds the Organizers or Sponsors, their representatives and successors, free and clear of any liability for any loss, injury or any other circumstances which may occur during the Event.

I agree that in the event of race cancellation due to storm, rain, inclement weather, wind or any other unforeseeable, or “act of God” conditions, my entry fee shall be non-refundable.
